The PFL Weekly Reporting form is due before 1 p.m. (ET) each Sunday during the season, whether your team plays or not.

Please fill out the following form completely, particularly supporting information for Player of the Week nominations. The information you include on this weekly form is the starting point for the weekly capsules in the weekly notes package.

In addition, please e-mail a headshot of your nominated player(s) to [email protected] before 3 p.m. (ET), Sunday for use in the weekly report and on the website. You do not need to email headshots if you have uploaded them to Dropbox.
